By accessing or using the services of Care Route Logistics LLC ("Company," "we," "us," or "our"), including our website, mobile drug and alcohol testing services, blood draw services, notary services, and training programs, you agree to be bound by these Terms and Conditions. If you do not agree, please do not use our services.
Care Route Logistics LLC provides mobile and on-site drug and alcohol testing (DOT and non-DOT), blood draw and phlebotomy services, notary services, and professional training programs including DOT Collector Certification, CDL General Knowledge Preparation, and Dispatcher Training. All services are performed by certified and trained professionals in accordance with applicable federal, state, and local regulations.
All DOT-regulated drug and alcohol testing services are conducted in strict compliance with 49 CFR Part 40 (U.S. Department of Transportation Procedures for Transportation Workplace Drug and Alcohol Testing Programs) and all applicable agency-specific regulations including FMCSA, FAA, FTA, FRA, PHMSA, and USCG. Our collection procedures, chain of custody documentation, and reporting processes adhere fully to federal requirements. Employers and employees subject to DOT testing are responsible for understanding their obligations under applicable DOT agency regulations.
Care Route Logistics LLC is committed to protecting the confidentiality of all test results and personal information. Drug and alcohol test results are disclosed only to authorized parties as permitted or required by 49 CFR Part 40, applicable DOT agency regulations, court order, or with the written consent of the tested individual. Non-DOT test results are handled with the same standard of confidentiality. We do not sell, rent, or share personal information with third parties for marketing purposes.
All training course materials, curricula, and instructional content provided by Care Route Logistics LLC are developed and maintained in accordance with applicable agency policies and procedures, including but not limited to DOT/FMCSA regulations, 49 CFR Part 40, and industry best practices established by the Drug and Alcohol Testing Industry Association (DATIA) and other recognized professional bodies. Course content is reviewed and updated regularly to reflect current regulatory requirements. Certificates of completion are issued upon successful fulfillment of course requirements and do not constitute a government-issued license or certification unless otherwise specified.
Payment is due at the time of service unless a prior arrangement has been made. Training program deposits are non-refundable unless the course is cancelled by Care Route Logistics LLC. Remaining balances for training programs are due prior to the course start date. After-hours service rates apply to all callouts between 8:00 PM and 6:00 AM. A mileage surcharge of $0.70 per mile applies to travel beyond 25 miles from the nearest hub city (Amarillo, Lubbock, Clovis, Dalhart, or Tucumcari).
Cancellations of scheduled testing appointments must be made at least 2 hours in advance to avoid a cancellation fee. Post-accident and reasonable suspicion testing callouts are non-cancellable once dispatched. Training enrollment cancellations made more than 7 days before the course start date may be eligible for a credit toward a future course. Cancellations within 7 days of the start date forfeit the deposit.
Care Route Logistics LLC shall not be liable for any indirect, incidental, special, or consequential damages arising from the use of our services. Our total liability for any claim arising from services rendered shall not exceed the amount paid for the specific service giving rise to the claim. We are not responsible for employment decisions made by employers based on drug or alcohol test results.
Clients are responsible for providing accurate identification and information at the time of service. Providing false information, tampering with specimens, or attempting to adulterate or substitute a specimen is a violation of federal regulations and may result in a refusal-to-test determination, which carries the same consequences as a positive test result under DOT regulations.
These Terms and Conditions are governed by the laws of the State of Texas. Any disputes arising from the use of our services shall be resolved in the courts of Potter County, Texas, or through binding arbitration as mutually agreed upon by the parties.
Care Route Logistics LLC reserves the right to update these Terms and Conditions at any time. Changes will be posted on this page with an updated effective date. Continued use of our services after changes are posted constitutes acceptance of the revised terms.
